The problem

Knowing the step is not proving it was done.

A procedure can be written, trained, and signed, and still not be carried out as intended. The gap that hurts is at the point of work, between what was attested and what actually happened. It is documented, and it is expensive.

A known step, never confirmed

The prototype of the Boeing B-17 stalled and crashed on take-off after the crew left the elevator gust locks engaged. The step was known. Nothing confirmed it had been done before the aircraft left the ground.

The US District Court found that misinterpretation of the negative pressure test, the critical test of well integrity, was a substantial cause of the Deepwater Horizon blowout, which killed eleven people in the Gulf of Mexico.
